This assessment tool and progress report is utilized by Mrs. Kathy Poszgai at West Park Elementary School in Ravenna, OH. It is sent home every semester with the students in their report cards. It enables both the teacher and the parent to see where a student is lacking in ability compared with other children his or her age.
